Expanding access to mobile care in Perry County

We aim to help make mobile care services available to Perry County residents. Mobile care services bring medical care, including screenings, dental care, substance use treatment, vision care, and other types of care directly to community members who need it by providing care in a van or other mobile setting.

 

This work began in the Fall of 2024 with a virtual convening of mobile care providers in the County. Based on what we learned about the services they provide, the community members they serve, and the successes and challenges they experience, we are now working with mobile care providers to help make it easier for community members to access high-quality, affordable mobile care services that meet their needs through awareness-raising actions and facilitating partnerships with mobile care providers.

Expanding access to mental health care in Perry County

We aim to help improve access to mental health care in Perry County by acting as a connector and convenor for mental health providers and services. This includes efforts to care for the social, emotional, and psychological well-being of community members, as well as efforts to prevent and treat mental illness. 

 

This work will began in early 2025 with a convening of mental health care providers in the County. From this meeting, we decided to connect community groups with mental health training programs, and we are providing information to increase mental health referrals via primary care provider pathways.
